What is Saline infusion sonography (SIS) ?

Saline infusion sonography (SIS), also known as saline sonohysterography or saline ultrasound, is a medical imaging procedure used to evaluate the inside of the uterus (the womb) and the uterine cavity. It is typically performed by a gynecologist or radiologist and is often used to diagnose or evaluate conditions such as:

• Uterine Abnormalities: SIS can detect structural abnormalities in the uterus, such as polyps, fibroids (benign growths), or uterine septa (a wall or partition within the uterus).
• Unexplained Uterine Bleeding: When a woman experiences abnormal or heavy uterine bleeding without an obvious cause, SIS can help identify the underlying issue.
• Recurrent Miscarriages: It can be used to investigate recurrent miscarriages and assess the uterine cavity for any abnormalities that might contribute to pregnancy loss.
• Infertility Evaluation: SIS is sometimes part of the workup for couples experiencing infertility, as uterine abnormalities can interfere with pregnancy.

Here's how SIS typically works:

• Preparation: The procedure is usually performed after a woman's menstrual period but before ovulation. The patient may be asked to take a pain reliever before the procedure. An empty bladder is also recommended for better imaging.
• Ultrasound Equipment: A transvaginal ultrasound probe (a specialized wand) is inserted into the vagina to obtain images of the uterus.
• Saline Injection: A thin catheter is inserted through the cervix into the uterine cavity. Then, a sterile saline (saltwater) solution is slowly injected into the uterus while the ultrasound is in progress. The saline expands the uterine cavity, allowing for better visualization of its interior.
• Imaging: The ultrasound images are captured in real-time as the saline fills the uterus. This allows the healthcare provider to assess the uterine lining and identify any abnormalities.
• Post-Procedure: After the procedure, the saline is drained from the uterus, and the catheter is removed. The patient can typically resume normal activities shortly afterward.

• Diagnosis of Abnormal Uterine Bleeding: In cases of abnormal uterine bleeding, SIS can help identify the cause. It can distinguish between structural abnormalities, hormonal issues, or other underlying conditions that may be contributing to the bleeding.
• Guidance for Treatment Planning: Once abnormalities or issues are detected through SIS, doctors can develop personalized treatment plans. For example, if uterine polyps are found, they can be removed surgically. If fibroids are identified and are causing symptoms, treatment options can be discussed.
• Infertility Evaluation: SIS is often part of the diagnostic workup for couples experiencing infertility. Identifying uterine abnormalities or structural issues can help determine if they are contributing to fertility problems. If so, appropriate interventions can be planned.
• Assessment of Postoperative Changes: SIS can be used to assess the uterine cavity after certain gynecological surgeries, such as myomectomy (removal of fibroids) or uterine septum resection. It helps ensure that the surgical site has healed correctly and that no new issues have arisen.
• Follow-Up and Monitoring: In some cases, SIS may be used for follow-up imaging after treatment to ensure that any identified issues have been successfully addressed.
